The fact that via accelerometry an effect was measurable at the central tremor peak, but not the overall tremor power between 2 and 15 Hz in the nonweighted condition could be a further argument for a specific CNS effect. The study failed to meet its primary efficacy outcome at 80 minutes after administration, at which time a maximum effect was anticipated. However, an effect of OA was measurable later than expected, and was more pronounced when tremor power of both hands were analyzed together, suggesting a systemic drug effect. Because a single time point was chosen as primary outcome to describe the peak effect of this low dose, future trials examining the therapeutic benefit in an outpatient setting should assess longer-term effects of OA. In an open label prospective trial, 37 patients received olanzapine, most taking mg daily in divided doses.140 Tremor significantly improved, and the effect was maintained over six months. Sedative side effects lessened one week after the start of treatment.

The longer effect latency to ET tremor suppression suggests a site of action with relatively isolated bioavailability, such as the CNS. However, this apparent anomaly can be explained by the presence of a blood–tissue barrier that surrounds extrafusal muscle spindles, a proposed peripheral site of action for beta-blockers. Alternatively, CNS beta-2 blockade may help diminish tremor by reducing CNS norepinephrine release or other mechanisms. One recent study found beta-blocker-responsive ET subjects had a greater reduction in CNS glucose metabolism compared with non-responsive subjects.70 Since all beta-blockers enter the CNS to some degree, contribution from a CNS site of action cannot be entirely eliminated. We propose that the improvement of varied hyperkinetic movement disorders with modest doses of EtOH or GHB doesnotderive from a simple pharmacologic effect on the GABA-A, GABA-B or GHB receptors.
Management and Treatment
Neuroleptics or dopamine antagonists cause classic parkinsonian tremor. Data were captured using commercial software (NeuroScan, Herndon, VA). The continuous files were broken into 8,192-millisecond epochs at a sampling rate of 1,000 Hz. Total tremor power in the spectral peak was calculated and averaged across epochs using self-developed Matlab® Scripts. Patient #8 was videotaped in fifteen-minute intervals after ingesting 1.5 gm of Xyrem to assess the pharmacokinetics of the improvement. Patients with known flushing syndrome or allergic reactions to alcohol (determined by the Alcohol Flushing Questionnaire)28 were also excluded. Alcohol is known to improve the symptoms of ET because of its impact on some brain chemicals that doctors have identified as causing tremors. Although alcohol abuse and alcohol withdrawal are linked to other types of tremors, ET is not thought to be among them.
